STUDENT EXAMINATION DECLARATION FORM

As part of the requirement for First Semester Exams 2024/2025 Session, students are required to download, print, fill and sign the Exams Declaration Form at the link below. Thereafter, students are required to submit signed copy of the Exams Declaration Form to their dept and bring along copy to the Exams venue. This is in addition to the requirements for Exams Cards and ID Cards.
